Look for “Index No.” or “Index Number”

A New York court document may label the case identifier as “Index No.” or “Index Number.” Copy it exactly, including the year. Do not shorten it or leave off digits.

  1. Check the top portion of the documentReview the caption area of the summons, complaint, notice or another filed document.
  2. Copy the entire numberInclude the slash and year when shown, such as 612345/2026.
  3. Add the countyThe same or similar number may relate to another county or case.
  4. Add the matching name and addressThese details help confirm that the record belongs to the correct homeowner and property.

What if I cannot find it?

You may still use the property-address option on the main form. Provide the full name, property address and county. Locating the correct record may take longer and could require manual verification.

Remember: A public-record review is not legal advice and does not change a court deadline. Seek prompt assistance from a New York-licensed attorney when a deadline, sale or appearance is close.
